109學年度轉系、所申請公告Application for Changing Major in Academic Year 2020

Application for Changing Major in  Academic Year 2020
 
 申請時間：109年5月19日起至6月16日截止
         
 申請流程：
    （一） 申請表請至「校務資訊系統」填寫後將紙本印出
                （系統於5月19日開放）
                南大校區學生(105學年度前入學)請至下列網址下載申請表：
    （二） 學士班：送請原就讀學系、院學士班主任及導師簽註意見。
    （三） 研究生：送請原就讀系、所、學位學程指導教授及主管簽章。
    （四） 學士班學生申請轉系須繳交各學系、院學士班之審查資料說明，請參照參考資料第2點所列之各系網址。
    （五） 請於6月16日前將轉系所申請表送達註冊組辦公室，學士班審查資料送達欲申請的各學系、院學士班辦公室，逾期不受理。
              （南大校區學生請將申請表送至南大校區註冊組辦公室）。
 
 備註：
 1、自103學年度取消各學系(院學士班)轉系(組、院學士班)審查標準。
 2、不得申請轉系之身份： 
 (1) 依台灣聯合大學系統學生轉校規定核准轉入本校就讀者，不得申請轉系、組、院學士班。
 (2) 依離島生資格入學者，依據「離島地區學生保送高級中等以上學校辦法」第十條規定，不得申請轉校（系、科）。但有特殊情況報經原保送之地方政府同意者，不在此限。
 (3) 依申請入學管道入學之公費生。
 (4) 依學士後第二專長學位學程規定入學者。
 3、陸生轉系(所)以入學至申請轉系(所)期間，各學年度曾經教育部核定本校得招收陸生之院系所、學位學程為限。
 4、轉系(所)核准名單將於8月下旬於本校註冊組網頁公告，因轉系(所)前後學雜費(基數)可能不同，請先暫緩繳納下學期學雜費(基數)，待核准名單公告及註冊組通知後再行繳費。

Ⅰ．Time： From 2020/05/19 to 2020/06/16
Ⅱ．Process：
    A．Please go to the Academic Information Systems to fill in the application form and print it out.
           Students on Nanda campus who were enrolled before Nov.1, 2016 shall go to the following website to download the application form:
    B．For undergraduate students: Bring the application form to your original department. The application form should be duly signed by the Chair and the tutor. 
    C．For graduate students: Bring the application form to your original department. The application form should be duly signed by the Chair and the advising professor.  
    D．For undergraduate students, please refer to Note 6 to prepare the required application packets. 
    E．The application forms shall be sent to the Division of Registration before June 16, 2020, while the students on Nanda campus shall send their application forms to the Division of Registration on Nanda campus. For undergraduate students, the required application packets shall be sent to the intended department’s office before June 16, 2020 Late application will not be accepted.
 
NOTE:
A．Cancel the review criteria for each department from 2014 academic year.
C．Undergraduate students are required to hand in an application packet for the application process. Please refer to the following websites to look for the required information.
