Notice for Changing Major Application in Academic Year 2021 (Adjusting to be online applying)

Changing Major Application in Academic Year 2021 has been adjusted to be online application due to epidemic prevention.
Applicants should prepare the following two items and email all application documents to the staff member who is in charge of the affairs of your first-desiring-department or institute in the Division of Registration. For details, please refer to the contact emails on the website of Division of Registration.

A.  Application form with relative approval proofs to Division of Registration: 
dot  Required documents
1.    Electronic file 1: Students should log in NTHU Academic Information System  Changing Major Application. Please download your application form and save as a pdf. file.
2.    Electronic file 2: Please attach relative approval proofs of your corresponding emails with your tutor(adviser) and Dean of department(institute) with your application form.
       To avoid the possibility of becoming ignored or junk emails, tutors(advisers) and Deans of department(institute) can empower the staff member in the departments(institutes) to be deputies to take care of the approval and stamp matters.
   a. Undergraduate students: the tutors and the Deans of the department (or deputies) where the applicants belong to originally.
   b. Graduate students: the advisers and the Deans of the department/institute ( or deputies) where the applicants belong to originally.

dot Email’s subject: Student ID No./Applicant’s name/Name of your first-desiring-department (institute)
For example: 109999001/Bill Gates/Applying for the 2nd Grade of Double Specialty Program of Management and Technology.

dot Documents to the staff member of Division of Registration: Please collect the above two electronic files and email to the staff member who is in charge of your first-desiring-department (institute) in the Division of Registration during
    May  18th to June 15th.  Applicants should keep in good touch with their tutors(advisers) and Deans of department(institute) actively to get their transferring-out results in time. Delayed application will not be accepted.
For example: If you apply to transfer into Double Specialty Program of Management and Technology as your first choice and Department of Chinese Literature as your second choice, please email your application form and relative approval proofs to Ms. WEN, HSIU-YUN(email: hywen@mx.nthu.edu.tw) , who is in charge of the affairs of your first-desiring-department.

dot Within 5 working days from the day of the applicants emailing the completed application documents to the Division of Registration, they will receive the confirmed emails. If applicants don’t receive confirmed emails, please contact the Division of Registration actively.

B. Individual screening documents to the desiring departments/institutes: (If the applicant has two choices, he/she shall prepare completed documents to his/her desiring departments separately.) 
dot All individual screening documents should be emailed to the staff member of applicants’ desiring departments (institutes) directly. Professor’s recommendation letter shall be emailed to the staff member of the applicant’s desiring  department in the format of pdf. by the professor directly, if any. (Transcripts will be prepared by Division of Registration, if any.)
